Hospital to Home Transition
We can support patients in the hospital to home transition, whether it is a staged discharge or returning home permanently. We work closely with the service usehttp://https//kasecare.co.uk/hospital-to-home/r’s family, rehabilitation clinics, therapists and other healthcare professionals to make the change as smooth and supportive as possible.
Once the service user is home, our care and support does not end there. Our care team can arrange for home care visits and/or assisted living services as and where we required as the service user continues their rehabilitation at home. Not only does this ensure that the service user receives the highest level of care available, but it will also take much of the burden off family and friends who might otherwise feel responsible for administering care alongside their own responsibilities.
